To provide support to patients and practitioners across end state renal networks in the united states to seek to assure healthcare security for patients. this includes assistance for access to equitable, appropriate and quality patient and family centered health care that achieves desired outcomes, protection of rights and dignity, consumer satisfaction, and dissemination of clear and useful information to assist with healthcare decisions. in concert with end stage renal networks and their missions, the foundation's strategic goals are (i) to improve the access for patients to quality and safety of dialysis related services provided for individuals, (ii) to help with the independence and quality of life of individuals through use of self-care modalities (such as transplantation, peritoneal dialysis, home hemodialysis), and (iii) to support self-directed care, as appropriate, through the end of life. in furtherance of the foregoing, the foundation will provide education, support and advocacy for patients, caregivers and professionals to support qualify of care and life of people with chronic kidney disease. the activities of the foundation are charitable, educational and scientific within the meaning of section 501(c)(3) of the code.
