In 2024, we provided over 3025 clinic and behavioral health visits with onepractitioner and one mental health clinician. over 80% of our patients thispast year were of hispanic and latino descent with almost 45% beinguninsured. almost 50% live with chronic conditions such as diabetes andheart disease.Hec had been operating as a department of health and human services, healthresources and services administration (hrsa) section 330 community healthcenter (chc) having received $1 million dollars from hrsa for a servicearea competition (sac-aa) on 11/17/2020. as a new chc from sac-aa, theservice area was required to proceed through a sac application process (fy2022 service area announcement, hrsa-22-103) in the health center's firstyear of operation. hrsa's objective review committee final summarystatement scored our application with a raw score of 95, without thebenefit of additional priority points as a 1st year chc. hec was not fundedas it was determined another organization was awarded
