Community Health Coalition of Caldwell County

Improve access to and delivery of health services to residents of Caldwell County, Texas who are uninsured through assessment, intervention, prevention, screening and health promotion for the uninsured. The Coalition operates three distinct community programs that support these eight objectives: 1)expand access to primary care services and self-management education services for Caldwell County residents diagnosed with Type 1 or Type 2 diabetes; 2)increase enrollment of low income residents into the Patient Pharmacy Assistance Program (PPAP), which provides low income individuals access to prescription medications at little to no cost; 3) increase enrollment in public/private insurance programs; 4)decrease emergency room and inpatient visits for diabetes; and 5)decrease BMI in obese adults; 6)improve access to all available medical and social service programs; 7)improve access to transportation services for medical visits; and 8) provide free durable medical equipment to residents of th
