Adult family care - nonotuck resource associate's rns and care managers screen referred people to determine that they are masshealth eligible and meet the criteria of needing assistance with certain activities of daily living, such as bathing, eating, dressing, ambulation, etc. Upon acceptance into the program, the registered nurse provides medical case management and there is regular case management and clinical support provided at regularly scheduled home visits. This program is designed to support families to remain intact and to keep people out of institutional care. 940 people served in afc as of 06/30/2025.
