Neighborhood family practice (nfp) is a network of community health centers committed to delivering high-quality, affordable primary, preventive, and supplemental healthcare. since 1980, nfp has been serving families on the west side of cleveland. in 2023, nfp provided care to 22,140 patients of all ages, resulting in approximately 80,804 visits across medical, behavioral health, midwifery, and dental services. about 60% of nfp's patients live between 0% and 200% of the federal poverty level, are uninsured or on medicaid, and face socioeconomic challenges due to low income and limited health coverage. among nfp's patient population, 24% are hispanic/latinx, 19% are black, and 11% are asian, reflecting the diversity of the communities served across seven community health centers: ridge, ann b. reischman, tremont, detroit shoreway, puritas, w. 117th, and north coast, spanning twelve neighborhoods on the west side of cleveland. nfp serves as a medical home, where a physician-led care team collaborates closely with each patient to achieve positive health outcomes. this strong patient-care team relationship encourages patients to seek regular care, follow through with primary caregiver recommendations, and prioritize their healthleading to improved health status, reduced unnecessary emergency room visits, and better-quality outcomes. nfp's care teams focus on preventive care, chronic disease management, mental health, and substance abuse treatment, and also offer women's health, midwifery, refugee care, dental, and pharmacy services. nfp is accredited by the joint commission and recognized for high-quality outcomes, especially in chronic disease management. the north coast health foundation (nchf) was established in 2019 as a supporting organization of neighborhood family practice. the purpose of the nchf is to increase awareness of nfp services and to foster philanthropic support from individuals and corporations to support nfp operations and capital needs.
