these expenses also include the professional responsibility to design, create, integrate, interpret, and maintain the digital systems appropriate to cancerforwards particular and customized needs.Part iii, statement of program service accomplishmentsthe background of the organization:the concept of cancer survivorship, now widely accepted and addressed by the cancerresearch, prevention and treatment communities, does not have a long history in relative terms. most historical overviews of cancer survivorship generally date the first usage of the term to 1986, when the intellection captured the attention of those workingin the fields of cancer care by drawing distinction to the ongoing issues of cancer patients following treatment. upon cancerforwards inception in 2010, the seven largest healthcare institutions operating in the texas medical center issued a joint public statement to emphasiz the need and important of collaboration between healthcare institutions and public harities in developing cancer survivorship rehabilitation programs. signed by the chief executive officers of the seven institutions, the statement read: reflecting increased global awareness of the public health impact of cancer survivorship, there is rapid growth in rigorous scientific inquiry and the numbers of those engage in care that addresses the long-term needs of cancer survivors. never before has such a higher calling been placed on healthcare institutions and survivor-centered organizations to collaborate in developing effective cancer rehabilitation programs. that is why the proliferation of survivor-focused public charities like cancerforward is so important. we share the hope and belief that the most effective cancer survivorship programs will address the physical, emotional and practical living needs of survivors today and will promote, research, interventions and sharing of best practices, this reducing the long-term burden of cancer to the public. the primary exempt purpose of the organization:cancerforwards primary exempt purpose is captured in its stated mission is to connect, educate and share information among cancer survivors, empowering them to move forward with their lives. cancerforward defines a cancer survivor as anyone who has been diagnosed with cancer. family caregivers and friends of the diagnosed are also considered survivors. cancerforward defines cancer survivorship as beginning at the time of disease diagnosis and continuing throughout the remainder of the patients life. the population of diagnosed cancer survivors residing in the united states is growing annually and is expected to reach 18 million by 2020.Program service accomplishments of he organization:cancerforward managed, maintained and promoted its digital communications platforms to virtually deliver the bulk of its key program services designed for cancer survivors situated globally. these programs are continuously ongoing and are centralized at canerforward.Org. program 1: digital survivorship information resources: cancerforward provides survivors and caregivers a comprehensive online resource library of survivorship and caregiver support tools and practicums in shared collaboration with more than 450 cancer-related organizations. the library is updated continuously and is organized into cancer-specific and general-support categories. cancerforward also provides as an online resource a calendar of local, regional and national opportunities where survivors can participate in survivorship clinical trials, research studies, bricks and mortar support groups, and seminars/webinars on employment, insurance, health care, and ada issues, to name a few. additionally, cancerforward guides survivors to sources where they can review a variety of survivorship plans for discussion with their primary healthcare provider.Program 2: digital survivorship education: cancerforward provides to survivors and caregivers online expert guidance on cancersurvivorship planning and cancer rehabilitation with educational content recommended by a national editorial advisory board of 30 cancer survivorship experts. the organization provides practical-use articles written exclusively for and available at no cost to cancerforward subscribers by leading u.S. oncologists and survivorship specialists, which include oncology certified nurses, psychosocial oncologists and social workers, mindfulness counselors, theologians, certified oncology nutritionists, and clinical exercise physiologists. cancer survivor stories are also provided as educationalresources. studies have shown that these stories help dispel myths, reduce stigma, andempower all survivors to maintain hope and open communications with medical personnel, family and friends.Part iv list of officers, directors, trustees and key employeesthe listing of parsons in this section is responsive to the requirement that the table becompleted
