Over the past year there has been a 53% rate of reunification with their parents which is approximate 253 children and another 150 found permanent homes with other family members. Cfc experienced a slight rise in the number of children in out of home care by slightly less than 3% in the last 12 months.\n\nMaintenance & adoption subsidy:cfc disburses maintenance adoption subsidy payments for approximately 2,000 children. 50% of our adoptions are completed within 24 months of a child's removal and this has exceeded cfcs goal for the past 4 years. Cfc finalized 137 adoptions during this year.\n\nIndependent living:cfc's contractor provides services to 180 former foster youth through this program which includes not only life coaching and skills training, but also offers financial support in the form of monthly stipends and emergency assistance. Over the course of the year, over 900 children are served for a total service day count of about 180,000. Cfc contracts with a variety of licensed child placing agencies that are able to address the unique needs of each child. Maltreatment while in licensed care or 6 months after is low for cfc, with 96.8% of our children not experiencing re-abuse while in care.\n\nCase management:cfc's case management services are provided to well over 2,000 children and their families each year. At the beginning of the fiscal year, cfc contracted with two case management organizations to serve the population in palm beach county. During the year, cfc made the decision to the north and west county case management activities in-house with the goal of improving services. This change was effective on march 1, 2012. 85.2% of the children in out-of-home care had two or fewer placements which is a key indicator of stability for the children.\n\nMaintenance & adoption subsidy:cfc disburses maintenance adoption subsidy payments for approximately 2,000 children. More than 50% of our adoptions are completed within 24 months of a child's removal and this has exceeded cfcs goal of 137 adoptions for this year and consistenty exceeded the goal for the past 4 years. Cfc finalized 151 adoptions during this year.\n\nIndependent living:cfc's contractor provides services to 190 former foster youth through this program which includes not only life coaching and skills training, but also offers financial support in the form of monthly stipends and emergency assistance. In addition, the program serves all youth in foster care ages 13-17 by preparing them for educational and vocational goals and life skills they need to be successful adults. 48.2% of these youth have earned their ged or high school diploma. The statewide average for this measure if 40%.diversion program:these services provide intensive in-home services to prevent the removal of children from the home.
