The program is designed to serve clients in need of 24-hour supervision due to significant disabilities in social roles and daily living functioning. Services within the setting are comprehensive but individually tailored to client needs. The goal is to assist individuals suffering from severe mental illness to move from hospitalization to residence in the community and to increase their skills to live as independently as possible.\n\nThe pre-trial education program provides psycho-educational classes to individuals arrested for driving while impaired or minor possession of drugs or drug paraphernalia. First time offenders are enrolled in an 8, 10, or 15 week educational program to address the problems of drinking and driving or substance abuse. Participants are referred by the bail commission or the office of adult probation and are then enrolled in a group interaction module. Pendelton house is a program designed to provide transitional housing and rehabilitation support to homeless individuals suffering from prolonged mental illness along with substance abuse. A shared housing arrangement offers an opportunity for clients to learn to manage their illness more effectively and to increase their skills of independence. The milieu of the program community provides structure and support for maintaining sobriety. Case management services also help clients access resources fo the mental health system and community at large. Also-cornerstone, inc. Offers a department of public health licensed outpatient clinic serving adults and families with mental illness, substance abuse, or co-occuring disorders. The clinic is also designated, as an enhanced care clinic for families and contracts with the agency on aging to serve elderly personal with mental health treatment needs. Clinicians are trained and licensed in a variety of behavioral health disciplines. The program of norton court offers a range of supportive and rehabilitative services to invidividuals with mental illness who are attempting to reestablish their residence following a period of hospitalization. Depending on the invidivual needs of the client, services may include assistance in locating and maintaining an apartment, enhancing skills in the management of a daily life routine, accessing vocational and recereational resources in the community, social role modeling, individual assessment and goal directed counseling and crisis intervention. The ruoppolo manor supportive housing program is operated in collaboration with the housing authority of new haven. Program services enable residents of ruoppolo manor to succeed in public housing. The program targets individuals who have been homeless or at risk of becoming homeless and who have either a dual diagnosis of mental illness and substance abuse, or a history of substance abuse with at least a year in treatment. Intensive case management services are directed toward a designated number of high- need individuals while supportive services are available to all residents of the building. The prevention program is an outreach and multi-family group process designed to help children succeed at home, in school and in society. Its goals are to strengthen families, reduce stress, enhance child resilience and reduce substance abuse in children and families. Fast is grounded in family stress theory, family systems theory and community development. It applies research-based strategies and has been empirically validated with outcome evaluations. Beginning awareness basic education studies (babes) is a nationally recognized primary prevention program designed to help children and young people develop positive living skills and provide them with accurate, non- judgmental information about the use and abuse of tobacco, alcohol and drugs. The robert t. Wolfe project is operated in collaboration with the housing authority of new haven serving 25 individuals in residence at this public housing complex. The program is designed to serve clients in need of 24-hour supervision due to significant disabilities in social roles and daily living functioning. Services within the setting are comprehensive but individually tailored to client needs. The goal is to assist individuals suffering from severe mental illness to move from hospitalization to residence in the community and to increase their skills to live as independently as possible.\n\nThe pre-trial education program provides psycho-educational classes to individuals arrested for driving while impaired or minor possession of drugs or drug paraphernalia. First time offenders are enrolled in an 8, 10, or 15 week educational program to address the problems of drinking and driving or substance abuse. Participants are referred by the bail commission or the office of adult probation and are then enrolled in a group interaction module. Pendelton house is a program designed to provide transitional housing and rehabilitation support to homeless individuals suffering from prolonged mental illness along with substance abuse. A shared housing arrangement offers an opportunity for clients to learn to manage their illness more effectively and to increase their skills of independence. The milieu of the program community provides structure and support for maintaining sobriety. Case management services also help clients access resources fo the mental health system and community at large. Also-cornerstone, inc. Offers a department of public health licensed outpatient clinic serving adults and families with mental illness, substance abuse, or co-occuring disorders. The clinic is also designated, as an enhanced care clinic for families and contracts with the agency on aging to serve elderly personal with mental health treatment needs. Clinicians are trained and licensed in a variety of behavioral health disciplines. The program of norton court offers a range of supportive and rehabilitative services to invidividuals with mental illness who are attempting to reestablish their residence following a period of hospitalization. Depending on the invidivual needs of the client, services may include assistance in locating and maintaining an apartment, enhancing skills in the management of a daily life routine, accessing vocational and recereational resources in the community, social role modeling, individual assessment and goal directed counseling and crisis intervention. The ruoppolo manor supportive housing program is operated in collaboration with the housing authority of new haven. Program services enable residents of ruoppolo manor to succeed in public housing. The program targets individuals who have been homeless or at risk of becoming homeless and who have either a dual diagnosis of mental illness and substance abuse, or a history of substance abuse with at least a year in treatment. Intensive case management services are directed toward a designated number of high- need individuals while supportive services are available to all residents of the building. The prevention program is an outreach and multi-family group process designed to help children succeed at home, in school and in society. Its goals are to strengthen families, reduce stress, enhance child resilience and reduce substance abuse in children and families. Fast is grounded in family stress theory, family systems theory and community development. It applies research-based strategies and has been empirically validated with outcome evaluations. Beginning awareness basic education studies (babes) is a nationally recognized primary prevention program designed to help children and young people develop positive living skills and provide them with accurate, non- judgmental information about the use and abuse of tobacco, alcohol and drugs. The robert t. Wolfe project is operated in collaboration with the housing authority of new haven serving 25 individuals in residence at this public housing complex. The outreach and engagement project is an interagency endeavor including also-cornerstone, inc., columbus house, connecticut mental health center, fellowship house, hill health corporation and marrakech behavioral health services. The goal of this program is to identify and engage eligible homeless indiviuals and provide a range of community-based clinical, case management and rehabilitative services to assist them in attaining greater community stability, and facilitate their smooth transitiion to continuing treatment and rehabilitative services. Provides a range of supported housing and case management services to individuals and families with psychiatric and substance abuse service needs living in the greater new haven community who are not eligible for dmhas funding. These services are supported through the united way, community foundations, contracts with other agencies, and private gifts and contributions generated through the company's fundraising efforts. THE CLINIC IS ALSO DESIGNATED, AS AN ENHANCED CARE CLINIC FOR FAMILIES AND CONTRACTS WITH THE AGENCY ON AGING TO SERVE ELDERLY PERSONAL WITH MENTAL HEALTH TREATMENT NEEDS. CLINICIANS ARE TRAINED AND LICENSED IN A VARIETY OF BEHAVIORAL HEALTH DISCIPLINES. THE PROGRAM OF NORTON COURT OFFERS A RANGE OF SUPPORTIVE AND REHABILITATIVE SERVICES TO INVIDIVIDUALS WITH MENTAL ILLNESS WHO ARE ATTEMPTING TO REESTABLISH THEIR RESIDENCE FOLLOWING A PERIOD OF HOSPITALIZATION. DEPENDING ON THE INVIDIVUAL NEEDS OF THE CLIENT, SERVICES MAY INCLUDE ASSISTANCE IN LOCATING AND MAINTAINING AN APARTMENT, ENHANCING SKILLS IN THE MANAGEMENT OF A DAILY LIFE ROUTINE, ACCESSING VOCATIONAL AND RECEREATIONAL RESOURCES IN THE COMMUNITY, SOCIAL ROLE MODELING, INDIVIDUAL ASSESSMENT AND GOAL DIRECTED COUNSELING AND CRISIS INTERVENTION. THE RUOPPOLO MANOR SUPPORTIVE HOUSING PROGRAM IS OPERATED IN COLLABORATION WITH THE HOUSING AUTHORITY OF NEW HAVEN. PROGRAM SERVICES ENABLE RESIDENTS OF RUOPPOLO MANOR TO SUCCEED IN PUBLIC HOUSING. THE PROGRAM TARGETS INDIVIDUALS WHO HAVE BEEN HOMELESS OR AT RISK OF BECOMING HOMELESS AND WHO HAVE EITHER A DUAL DIAGNOSIS OF MENTAL ILLNESS AND SUBSTANCE ABUSE, OR A HISTORY OF SUBSTANCE ABUSE WITH AT LEAST A YEAR IN TREATMENT. INTENSIVE CASE MANAGEMENT SERVICES ARE DIRECTED TOWARD A DESIGNATED NUMBER OF HIGH- NEED INDIVIDUALS WHILE SUPPORTIVE SERVICES ARE AVAILABLE TO ALL RESIDENTS OF THE BUILDING. THE PREVENTION PROGRAM IS AN OUTREACH AND MULTI-FAMILY GROUP PROCESS DESIGNED TO HELP CHILDREN SUCCEED AT HOME, IN SCHOOL AND IN SOCIETY. ITS GOALS ARE TO STRENGTHEN FAMILIES, REDUCE STRESS, ENHANCE CHILD RESILIENCE AND REDUCE SUBSTANCE ABUSE IN CHILDREN AND FAMILIES. FAST IS GROUNDED IN FAMILY STRESS THEORY, FAMILY SYSTEMS THEORY AND COMMUNITY DEVELOPMENT. IT APPLIES RESEARCH-BASED STRATEGIES AND HAS BEEN EMPIRICALLY VALIDATED WITH OUTCOME EVALUATIONS. BEGINNING AWARENESS BASIC EDUCATION STUDIES (BABES) IS A NATIONALLY RECOGNIZED PRIMARY PREVENTION PROGRAM DESIGNED TO HELP CHILDREN AND YOUNG PEOPLE DEVELOP POSITIVE LIVING SKILLS AND PROVIDE THEM WITH ACCURATE, NON- JUDGMENTAL INFORMATION ABOUT THE USE AND ABUSE OF TOBACCO, ALCOHOL AND DRUGS. THE ROBERT T. WOLFE PROJECT IS OPERATED IN COLLABORATION WITH THE HOUSING AUTHORITY OF NEW HAVEN SERVING 25 INDIVIDUALS IN RESIDENCE AT THIS PUBLIC HOUSING COMPLEX. ON-SITE STAFF PROVIDES A BROAD RANGE OF CASE MANAGEMENT SERVICES TO HIGH-NEED INDIVDUALS WHILE OFFERING SUPPORT TO ALL RESIDENTS OF THE BUIDLING. THE PROGRAM TARGETS INDIVIDUALS WHO HAVE BEEN HOMELESS OR AT RISK FOR BECOMING HOMELESS AND WHO HAVE EITHER A SUBSTANCE ABUSE HISTORY OR CO-OCCURING MENTAL ILLNESS AND ADDICTION. THE OUTREACH AND ENGAGEMENT PROJECT IS AN INTERAGENCY ENDEAVOR INCLUDING ALSO-CORNERSTONE, INC., COLUMBUS HOUSE, CONNECTICUT MENTAL HEALTH CENTER, FELLOWSHIP HOUSE, HILL HEALTH CORPORATION AND MARRAKECH BEHAVIORAL HEALTH SERVICES. THE GOAL OF THIS PROGRAM IS TO IDENTIFY AND ENGAGE ELIGIBLE HOMELESS INDIVIUALS AND PROVIDE A RANGE OF COMMUNITY-BASED CLINICAL, CASE MANAGEMENT AND REHABILITATIVE SERVICES TO ASSIST THEM IN ATTAINING GREATER COMMUNITY STABILITY, AND FACILITATE THEIR SMOOTH TRANSITIION TO CONTINUING TREATMENT AND REHABILITATIVE SERVICES. ALSO-CORNERSTONE, INC. PROVIDES A RANGE OF SUPPORTED HOUSING AND CASE MANAGEMENT SERVICES TO INDIVIDUALS AND FAMILIES WITH PSYCHIATRIC AND SUBSTANCE ABUSE SERVICE NEEDS LIVING IN THE GREATER NEW HAVEN COMMUNITY WHO ARE NOT ELIGIBLE FOR DMHAS FUNDING. THESE SERVICES ARE SUPPORTED THROUGH THE UNITED WAY, COMMUNITY FOUNDATIONS, CONTRACTS WITH OTHER AGENCIES, AND PRIVATE GIFTS AND CONTRIBUTIONS GENERATED THROUGH THE COMPANY'S FUNDRAISING EFFORTS. SUCH SERVICES INCLUDE THE FAMILY SUPPORT COLLABORATIVE, THE PARK STREET RESIDENCE COMMUNITY SERVICE PROGRAM, ALCOHOL SERVER TRAINING PROGRAM (TIPS) AND OTHER CHARITABLE AND REHABILITATIVE PROGRAMS. THE WEST VILLAGE PROGRAM IS A PARTNERSHIP BETWEEN ALSO-CORNERSTONE AND ALPHA COMMUNITY SERVICES, A BRANCH OF THE CENTRAL CONNECTICUT YMCA. THE APARTMENTS ARE MANAGED BY COMMUNITY BUILDERS OF NEW HAVEN AND THE PROGRAM IS FUNDED BY THE DEPARTMENT OF MENTAL HEALTH AND ADDICTION SERVICES. THE PROGRAM OFFERS REHABILITATIVE SERVICES TO 15 INDIVIDUALS WHO ARE HOMELESS OR AT RISK OF BECOMING HOMELESS AND WHO HAVE A DIAGNOSIS OF MENTAL ILLNESS AND/OR SUBSTANCE ABUSE, OR A HISTORY OF SUBSTANCE ABUSE. SUPPORTIVE HOUSING DEVELOPMENT SPECIALISTS RESEARCH AND IDENTIFY HOUSING RESOURCES AVAILABLE TO INCREASE THE NUMBER OF UNITS FOR CLIENTS INCLUDING BUT NOT LIMITED TO: NEW SUBSIDIZED SUPPORTIVE HOUSING UNITS, A LIST OF LANDLORDS, INFORMATION ON RENTAL SUBSIDIES AND GUIDELINES FOR PASSING STATE AND LOCAL INSPECTION STANDARDS WITHIN A ONE YEAR PERIOD.
