As a coalition, the AFCC provides or makes referrals to needed services, utilizing established partnerships with health care providers, schools and other resources, while supporting women within their home communities.When a client calls, emergency needs are addressed immediately. A professional case manager located as close as possible to the client's residence is assigned. Upon assignment, an individual assessment is completed with each client to determine risk factors for abortion, utilizing the Individual Risk and Needs Assessment and the Initial Client Assessment. The client, working together with the case manager, identifies support systems such as church, community and/or friends, and determines a priority of needs.The PMN process is one of client-centered decision making. The client, with the case manager, develops a service plan (Individualized Pregnancy Continuation Plan). Every month the client and the case manager review the plan and based on the client's current needs, update the plan until the program is completed or the stated objectives have been achieved. The plan specifies available services and support system that will allow a healthy outcome to the pregnancy.The case manager refers the client to existing community resources to meet these needs. If no other resources can be accessed, services may be covered by this contract. Education regarding the available services and limitations of this program continues throughout the client's participation.Case managers maintain client specific documentation which describes each contact, lists the time involved in each activity, and if appropriate, the cost. Services will terminate when the plan has been completed, the eligibility period (one year post delivery) has ended, or the client withdraws from the program.Prenatal CarePrenatal care is a priority for every woman in this program. The client, together with the case manager, works to identify her needs and resources (e.g. private insurance, Medicaid, private pay, etc.). The case manager makes referrals to local prenatal care providers, including doctors, midwives, health centers, clinics, and hospitals, for prenatal care only and prenatal care/delivery.Medical CareHealth maintenance, prevention, and treatment are provided by a licensed medical provider, as related to the prevention of abortion.Mental Health CareCounseling or treatment to assist the client with management of mental illness or other issues affecting her mental/emotional well-being is provided when indicated.Parenting SkillsEducation and counseling teach and develop the parenting role and assist mothers and fathers in being responsible parents.Drug and Alcohol Testing and TreatmentDrug/alcohol testing and treatment is provided as needed. When a problem is identified through report or through testing, the client is referred to an individual, group, or family program that is appropriately licensed or certified.Child CareThe client is assisted with child care arrangements when needed to participate in contract activities or services. A client may use either a day care facility or in-home care. Child care providers must meet minimum Children's Division standards, which require a Family Care Registry Screening.Newborn or Infant CareMedical care and interventions by a licensed medical provider may be provided to the baby throughout the first year of his or her life.HousingWhen needed, the client is assisted in finding appropriate safe housing. Depending on the age and needs of the client and the availability of resources, the following options will be considered: motel/hotel (for emergency situation), group shelter (including domestic violence shelters), transitional living, independent living, placement with a family, foster care, and residential treatment.UtilitiesAs established social services agencies, the subcontractors of this program have working relationships with the local utility companies and other community resources to assist with these rising costs. As a coalition, the afcc provides or makes referrals to needed services, utilizing established partnerships with health care providers, schools and other resources, while supporting women within their home communities.when a client calls, emergency needs are addressed immediately. A professional case manager located as close as possible to the client's residence is assigned. Upon assignment, an individual assessment is completed with each client to determine risk factors for abortion, utilizing the individual risk and needs assessment and the initial client assessment. The client, working together with the case manager, identifies support systems such as church, community and/or friends, and determines a priority of needs.the pmn process is one of client-centered decision making. The client, with the case manager, develops a service plan (individualized pregnancy continuation plan). Every month the client and the case manager review the plan and based on the client's current needs, update the plan until the program is completed or the stated objectives have been achieved. The plan specifies available services and support system that will allow a healthy outcome to the pregnancy.the case manager refers the client to existing community resources to meet these needs. If no other resources can be accessed, services may be covered by this contract. Education regarding the available services and limitations of this program continues throughout the client's participation.case managers maintain client specific documentation which describes each contact, lists the time involved in each activity, and if appropriate, the cost. Services will terminate when the plan has been completed, the eligibility period (one year post delivery) has ended, or the client withdraws from the program.prenatal careprenatal care is a priority for every woman in this program. The client, together with the case manager, works to identify her needs and resources (e.g. Private insurance, medicaid, private pay, etc.). The case manager makes referrals to local prenatal care providers, including doctors, midwives, health centers, clinics, and hospitals, for prenatal care only and prenatal care/delivery.medical carehealth maintenance, prevention, and treatment are provided by a licensed medical provider, as related to the prevention of abortion.mental health carecounseling or treatment to assist the client with management of mental illness or other issues affecting her mental/emotional well-being is provided when indicated.parenting skillseducation and counseling teach and develop the parenting role and assist mothers and fathers in being responsible parents.drug and alcohol testing and treatmentdrug/alcohol testing and treatment is provided as needed. When a problem is identified through report or through testing, the client is referred to an individual, group, or family program that is appropriately licensed or certified.child carethe client is assisted with child care arrangements when needed to participate in contract activities or services. A client may use either a day care facility or in-home care. Child care providers must meet minimum children's division standards, which require a family care registry screening.newborn or infant caremedical care and interventions by a licensed medical provider may be provided to the baby throughout the first year of his or her life.housingwhen needed, the client is assisted in finding appropriate safe housing. Depending on the age and needs of the client and the availability of resources, the following options will be considered: motel/hotel (for emergency situation), group shelter (including domestic violence shelters), transitional living, independent living, placement with a family, foster care, and residential treatment.utilitiesas established social services agencies, the subcontractors of this program have working relationships with the local utility companies and other community resources to assist with these rising costs. A professional case manager located as close as possible to the client's residence is assigned. Upon assignment, an individual assessment is completed with each client to determine risk factors for abortion, utilizing the individual risk and needs assessment and the initial client assessment. The client, working together with the case manager, identifies support systems such as church, community and/or friends, and determines a priority of needs.the pmn process is one of client-centered decision making. The client, with the case manager, develops a service plan (individualized pregnancy continuation plan). Every month the client and the case manager review the plan and based on the client's current needs, update the plan until the program is completed or the stated objectives have been achieved. The plan specifies available services and support system that will allow a healthy outcome to the pregnancy.the case manager refers the client to existing community resources to meet these needs. If no other resources can be accessed, services may be covered by this contract. THE PLAN SPECIFIES AVAILABLE SERVICES AND SUPPORT SYSTEM THAT WILL ALLOW A HEALTHY OUTCOME TO THE PREGNANCY.THE CASE MANAGER REFERS THE CLIENT TO EXISTING COMMUNITY RESOURCES TO MEET THESE NEEDS. IF NO OTHER RESOURCES CAN BE ACCESSED, SERVICES MAY BE COVERED BY THIS CONTRACT. EDUCATION REGARDING THE AVAILABLE SERVICES AND LIMITATIONS OF THIS PROGRAM CONTINUES THROUGHOUT THE CLIENT'S PARTICIPATION.CASE MANAGERS MAINTAIN CLIENT SPECIFIC DOCUMENTATION WHICH DESCRIBES EACH CONTACT, LISTS THE TIME INVOLVED IN EACH ACTIVITY, AND IF APPROPRIATE, THE COST. SERVICES WILL TERMINATE WHEN THE PLAN HAS BEEN COMPLETED, THE ELIGIBILITY PERIOD (ONE YEAR POST DELIVERY) HAS ENDED, OR THE CLIENT WITHDRAWS FROM THE PROGRAM.PRENATAL CAREPRENATAL CARE IS A PRIORITY FOR EVERY WOMAN IN THIS PROGRAM. THE CLIENT, TOGETHER WITH THE CASE MANAGER, WORKS TO IDENTIFY HER NEEDS AND RESOURCES (E.G. PRIVATE INSURANCE, MEDICAID, PRIVATE PAY, ETC.). THE CASE MANAGER MAKES REFERRALS TO LOCAL PRENATAL CARE PROVIDERS, INCLUDING DOCTORS, MIDWIVES, HEALTH CENTERS, CLINICS, AND HOSPITALS, FOR PRENATAL CARE ONLY AND PRENATAL CARE/DELIVERY.MEDICAL CAREHEALTH MAINTENANCE, PREVENTION, AND TREATMENT ARE PROVIDED BY A LICENSED MEDICAL PROVIDER, AS RELATED TO THE PREVENTION OF ABORTION.MENTAL HEALTH CARECOUNSELING OR TREATMENT TO ASSIST THE CLIENT WITH MANAGEMENT OF MENTAL ILLNESS OR OTHER ISSUES AFFECTING HER MENTAL/EMOTIONAL WELL-BEING IS PROVIDED WHEN INDICATED.PARENTING SKILLSEDUCATION AND COUNSELING TEACH AND DEVELOP THE PARENTING ROLE AND ASSIST MOTHERS AND FATHERS IN BEING RESPONSIBLE PARENTS.DRUG AND ALCOHOL TESTING AND TREATMENTDRUG/ALCOHOL TESTING AND TREATMENT IS PROVIDED AS NEEDED. WHEN A PROBLEM IS IDENTIFIED THROUGH REPORT OR THROUGH TESTING, THE CLIENT IS REFERRED TO AN INDIVIDUAL, GROUP, OR FAMILY PROGRAM THAT IS APPROPRIATELY LICENSED OR CERTIFIED.CHILD CARETHE CLIENT IS ASSISTED WITH CHILD CARE ARRANGEMENTS WHEN NEEDED TO PARTICIPATE IN CONTRACT ACTIVITIES OR SERVICES. A CLIENT MAY USE EITHER A DAY CARE FACILITY OR IN-HOME CARE. CHILD CARE PROVIDERS MUST MEET MINIMUM CHILDREN'S DIVISION STANDARDS, WHICH REQUIRE A FAMILY CARE REGISTRY SCREENING.NEWBORN OR INFANT CAREMEDICAL CARE AND INTERVENTIONS BY A LICENSED MEDICAL PROVIDER MAY BE PROVIDED TO THE BABY THROUGHOUT THE FIRST YEAR OF HIS OR HER LIFE.HOUSINGWHEN NEEDED, THE CLIENT IS ASSISTED IN FINDING APPROPRIATE SAFE HOUSING. DEPENDING ON THE AGE AND NEEDS OF THE CLIENT AND THE AVAILABILITY OF RESOURCES, THE FOLLOWING OPTIONS WILL BE CONSIDERED: MOTEL/HOTEL (FOR EMERGENCY SITUATION), GROUP SHELTER (INCLUDING DOMESTIC VIOLENCE SHELTERS), TRANSITIONAL LIVING, INDEPENDENT LIVING, PLACEMENT WITH A FAMILY, FOSTER CARE, AND RESIDENTIAL TREATMENT.UTILITIESAS ESTABLISHED SOCIAL SERVICES AGENCIES, THE SUBCONTRACTORS OF THIS PROGRAM HAVE WORKING RELATIONSHIPS WITH THE LOCAL UTILITY COMPANIES AND OTHER COMMUNITY RESOURCES TO ASSIST WITH THESE RISING COSTS. THE COST OF A MONTH OF UTILITIES IS BASED ON AN ESTIMATED, AVERAGE MONTHLY UTILITY COST INCURRED BY CLIENTS.EDUCATIONAL SERVICESREFERRALS ARE MADE TO LOCAL PUBLIC SCHOOL DISTRICTS AND GRADUATE EQUIVALENCY DIPLOMA (GED) PROGRAMS FOR WOMEN WHO HAVE NOT COMPLETED HIGH SCHOOL. OTHER RESOURCES FOR COLLEGE, VOCATIONAL AND TECHNICAL TRAINING MAY BE PURSUED AS APPROPRIATE.FOODFOOD PRODUCTS OR NUTRITIONAL PRODUCTS RELATING TO PREGNANCY, NEWBORN CARE, AND PARENTING MAY BE SUPPLIED.CLOTHINGCLOTHING RELATING TO PREGNANCY, NEWBORN CARE, AND PARENTING MAY BE SUPPLIED.SUPPLIESSUPPLIES RELATING TO PREGNANCY, NEWBORN CARE, AND PARENTING MAY BE PROVIDED.ADOPTION ASSISTANCESUBCONTRACTING AGENCIES PROVIDE EDUCATION, SUPPORT, AND CASE MANAGEMENT SERVICES REGARDING THE EMOTIONAL AND LEGAL ASPECTS OF ADOPTION. THE CLIENT IS REFERRED TO AN ATTORNEY WHEN INDICATED.JOB TRAINING AND PLACEMENTREFERRALS ARE MADE TO ESTABLISHED PROGRAMS THAT FACILITATE AND/OR ENHANCE THE EMPLOYABILITY OF THE CLIENT AND/OR FATHER OF THE BABY.EFFORTS TO PROMOTE RESPONSIBLE PATERNITYSERVICES TO THE FATHER MAY INCLUDE LEARNING OPPORTUNITIES REGARDING FATHER-CHILD INTERACTION, EDUCATION ON HOW TO ESTABLISH PATERNITY, PATERNITY TESTING, AND THE LEGAL PROCESS.ULTRASOUND SERVICESA CLIENT RECEIVES ULTRASOUND TESTS AS MEDICALLY INDICATED, UTILIZING HER OWN, OR PROGRAM REFERRED RESOURCES.DOMESTIC ABUSE PROTECTIONREFERRAL TO A DOMESTIC VIOLENCE AGENCY FOR SHELTER AND/OR EDUCATION ON PROTECTION IS MADE AS APPROPRIATE.
