(ghi), which is a minnesota non-profit corporation and licensed health maintenance organization recognized as exempt from federal income tax under internal revenue code (irc) section 501(c)(3). Ghi is also the sole corporate member of healthpartners research foundation, a minnesota non-profit corporation recognized as exempt from federal income tax under irc section 501(c)(3). Additionally, ghi is a sole corporate member of physicians neck & back clinics, a minnesota non-profit corporation that qualifies for tax-exemption under irc section 501(c)(3). Healthpartners, inc., a minnesota non-profit corporation and licensed health maintenance organization recognized as exempt from federal income tax under irc section 501(c) (4), is the sole corporate member of ghi, and the following organizations that are also exempt from federal income tax under irc section 501(c) (3): healthpartners institute for medical education, rhsc, inc. And hpi-ramsey. Hpi-ramsey, in turn, is the sole corporate member of regions hospital, regions hospital foundation, capital view transitional care center, lakeview health system, rh-wisconsin, inc., and ramsey integrated health services (a home care agency), all of which are non-profit corporations exempt from federal income tax under irc section 501(c)(3). Rh-wisconsin, inc. And ghi are corporate members of hudson hospital, inc. And westfields hospital, inc., both of which are wisconsin non-profit corporations exempt from federal income tax under irc section 501(c)(3). Rh-wisconsin, inc. Is also the sole corporate member of western wisconsin emergency medical services company, an ambulance service which is a wisconsin non-profit corporation exempt from federal income tax under irc section 501(c)(3). Together with the healthpartners family of exempt organizations, hpcmc seeks to be the best and most trusted provider of health care, health promotion, health care financing and health care administration in the united states. Acting in concert, this family of organizations is working to transform health care by delivering outstanding care and service that is consistent with the institute for healthcare improvement's \"triple aim\" initiative. The \"triple aim\" seeks to simultaneously optimize the health of the population, the experience of each individual and reduce per capita health care costs. Hpcmc, its parent organization ghi, and its sister organizations, healthpartners research foundation and physicians neck and back clinics, work together with the other exempt organizations within the healthpartners family of organizations to achieve these aims with maximum efficiency and collaboration. For example, when best practice, patient education materials, or system improvements are created by one entity, the benefits are spread to members and patients throughout the integrated system, as appropriate. Hpcmc and the healthpartners family of organizations also partner with other health plans, care providers, and non-profit organizations in the region and throughout the nation to increase access for the underserved, create and disseminate care improvement information, participate in the development of public policy, and maximize shared resources for the improvement of the health care system and the community's overall health. Ii. Provision of medical care hpcmc operates a clinic in sartell, minnesota. Approximately twenty-six percent of hpcmc's health care services are delivered to patients enrolled in programs operated by the state of minnesota, such as medicaid. Approximately fifteen percent of hpcmc's health care services are delivered to medicare program patients. The remaining fifty-nine percent of hpcmc's health care services are delivered to other patients, including those with commercial insurance. Approximately 63,898 visits were served at the clinic in 2011. Of these visits, approximately 47,937 had private insurance, 4,377 were covered by medicare, 2,926 were covered by medicaid and 7,351 held other public insurance (e.g. Medical assistance or minnesota care). The remaining 1,307 of the 63,898 were uninsured. In 2011, hpcmc provided $2,205,333 in uncompensated care charges (this included charity care, bad debt, and the difference in reimbursement between what medical assistance or medicare paid, and what would have been paid using commercial reimbursement rates). Hpcmc offers a full range of services. In addition to hpcmc's primary care services (family practice, pediatrics and obstetrics and gynecology), hpcmc offers its behavioral health center, dental center, pharmacy, chiropractic, lab and imaging, and specialty care coordination. Hpcmc also has two retail clinics called healthstations, one located in a grocery store and one in their main clinic and, a well at work clinic for employer groups. Iii. Other community benefit activities in 2011 to promote and improve the health of the community, hpcmc donates medical services, funds and provides internship placement sites. Additional areas of community benefit are described below. Community health education. Patients and family members can also access free health education materials during appointments using internet-capable computers available in hpcmc's main lobby, pediatrics lobby and the women's center. In addition, community members have access to health education through health fairs, children and women's expos, local schools, and worksite health promotions. Hpi-ramsey, in turn, is the sole corporate member of regions hospital, regions hospital foundation, capital view transitional care center, lakeview health system, rh-wisconsin, inc., and ramsey integrated health services (a home care agency), all of which are non-profit corporations exempt from federal income tax under irc section 501(c)(3). Rh-wisconsin, inc. And ghi are corporate members of hudson hospital, inc. And westfields hospital, inc., both of which are wisconsin non-profit corporations exempt from federal income tax under irc section 501(c)(3). Rh-wisconsin, inc. Is also the sole corporate member of western wisconsin emergency medical services company, an ambulance service which is a wisconsin non-profit corporation exempt from federal income tax under irc section 501(c)(3). Together with the healthpartners family of exempt organizations, hpcmc seeks to be the best and most trusted provider of health care, health promotion, health care financing and health care administration in the united states. Acting in concert, this family of organizations is working to transform health care by delivering outstanding care and service that is consistent with the institute for healthcare improvement's \"triple aim\" initiative. The \"triple aim\" seeks to simultaneously optimize the health of the population, the experience of each individual and reduce per capita health care costs. Hpcmc, its parent organization ghi, and its sister organizations, healthpartners research foundation and physicians neck and back clinics, work together with the other exempt organizations within the healthpartners family of organizations to achieve these aims with maximum efficiency and collaboration. For example, when best practice, patient education materials, or system improvements are created by one entity, the benefits are spread to members and patients throughout the integrated system, as appropriate. Hpcmc and the healthpartners family of organizations also partner with other health plans, care providers, and non-profit organizations in the region and throughout the nation to increase access for the underserved, create and disseminate care improvement information, participate in the development of public policy, and maximize shared resources for the improvement of the health care system and the community's overall health. Ii. Provision of medical care hpcmc operates a clinic in sartell, minnesota. Approximately twenty-six percent of hpcmc's health care services are delivered to patients enrolled in programs operated by the state of minnesota, such as medicaid. Approximately fifteen percent of hpcmc's health care services are delivered to medicare program patients. The remaining fifty-nine percent of hpcmc's health care services are delivered to other patients, including those with commercial insurance. Approximately 63,898 visits were served at the clinic in 2011. Of these visits, approximately 47,937 had private insurance, 4,377 were covered by medicare, 2,926 were covered by medicaid and 7,351 held other public insurance (e.g. Medical assistance or minnesota care). The remaining 1,307 of the 63,898 were uninsured. In 2011, hpcmc provided $2,205,333 in uncompensated care charges (this included charity care, bad debt, and the difference in reimbursement between what medical assistance or medicare paid, and what would have been paid using commercial reimbursement rates). Hpcmc offers a full range of services. In addition to hpcmc's primary care services (family practice, pediatrics and obstetrics and gynecology), hpcmc offers its behavioral health center, dental center, pharmacy, chiropractic, lab and imaging, and specialty care coordination. Hpcmc also has two retail clinics called healthstations, one located in a grocery store and one in their main clinic and, a well at work clinic for employer groups. Iii. Other community benefit activities in 2011 to promote and improve the health of the community, hpcmc donates medical services, funds and provides internship placement sites. Patients and family members can also access free health education materials during appointments using internet-capable computers available in hpcmc's main lobby, pediatrics lobby and the women's center.
