Services are designed to build strength and endurance, restore functional mobility, improve performance of daily activities and manage pain. More than 2,000 patients receive inpatient services each year at Bryn Mawr Rehabilitation Hospital for joint replacements, fractures, and amputations. They range in age from 18 to 97 years old. The average stay is eight days and ranges from 5 to 14 days. More than 90% of our inpatients return home after discharge. As a comprehensive rehabilitation facility, our services also include outpatient care for the treatment of strains, sprains, bursitis and sports injuries. We also offer a full complement of specialized services, such as orthotics and prosthetics, and Amputee clinic, pain management and aquatics. Orthopedic injuries and conditions are generally characterized by a decrease in strength, endurance, and mobility along with increased pain. Our treatment options concentrate on improving endurance and strength through a regimented exercise program, improving mobility with gait training, adaptive aids and alleviating pain.\n\nStroke (Neurology) Rehabilitation: Bryn Mawr Rehabilitation Hospital excels in meeting the complex rehabilitation needs of stroke patients. Experiences with thousands of patients have shown us that recovery is different for each person, depending on the area of the brain affected. We have developed a full continuum of lifetime care for people who have experienced a stroke. This system includes acute inpatient services, outpatient services, Day Treatment, community re-entry services, and Family Support Groups. When stroke survivors arrive at Bryn Mawr Rehabilitation Hospital, they usually require total or maximal assistance with daily activities. Our medical care, therapies, and special services help each person build on strengths and compensation for any limitations. Our stroke patients are between 42 and 97 years old, although patients as young as 18 have been admitted. Average inpatient stay is about three weeks, but can vary widely. Driver Rehabilitation Program Since 1983, Bryn Mawr Rehab Hospital's Driver Rehabilitation Program has provided objective driving evaluations, specialized equipment and training for people with disabilities or aging issues. Horticulture Therapy Program Horticultural Therapy is an innovative treatment modality that uses plants and plant-related activities to assist in the rehabilitation of people with disabilities. The Bryn Mawr Rehab Hospital Sydney Thayer III Horticultural Center is a state-of-the-art facility with cathedral ceilings, skylights and a greenhouse that creates a calm and therapeutic setting. Medical Rehab Program Bryn Mawr Rehab Hospital's Medical Rehabilitation Programs are designed to meet the rehabilitation needs of individuals who have a broad range of acute or chronic illnesses that cause temporary or permanent disability. Outpatient Programs With more than 28,000 outpatient visits each year, Bryn Mawr Rehab Hospital is one of the largest rehabilitation providers in the entire Philadelphia region. III. Main Line Hospitals, Inc. Main Line Hospitals, Inc. is a Pennsylvania nonprofit corporation. Main Line Health is the sole member of Main Line Hospitals, Inc. Main Line Hospitals, Inc. is the surviving entity of the 1996 merger of Lankenau Hospital and Paoli Memorial Hospital into Bryn Mawr Hospital. The Bryn Mawr Hospital corporation's name was changed following the merger to Main Line Hospitals, Inc. Effective July 1, 2010, Bryn Mawr Rehabilitation Hospital, formerly a separate Pennsylvania nonprofit corporation was merged into Main Line Hospitals, Inc. At present, Main Line Hospitals, Inc. operates Lankenau Hospital, Paoli Hospital and Bryn Mawr Hospital as separately licensed acute care hospitals, and Bryn Mawr Rehabilitation Hospital as a separately licensed acute rehabilitation hospital. Bryn Mawr Hospital, Lankenau Hospital, Paoli Hospital and Bryn Mawr Rehabilitation Hospital are governed by a Main Line Health board of governors consisting of 27 members of the community, including physician representation. The Board has overall governance and accountability patient safety and quality, and community outreach. Riddle Memorial Hospital has a separate 16member board of directors comprised primarily of community members, MLH management, and physicians. Each MLH hospital has an associated nonprofit Foundation engaged in fundraising and development. The Boards of each foundation consist of community representatives who provide oversight and guidance for the Foundations philanthropic activities. Bryn Mawr Hospital, established in 1893, is a community-based 311-bed acute care teaching hospital located in Bryn Mawr, Pennsylvania. Bryn Mawr Hospital offers a full complement of medical and surgical services, as well as specialized programs such as primary care services, an arthritis and orthopedic center, a comprehensive cancer car center, a cardiovascular center, and women's and children's health services. Bryn Mawr Hospital has the only in-patient psychiatric facility in Montgomery County as well as the only emergency department specific for children. The Bryn Mawr Hospital Health Center provides outpatient ambulatory care services in Newtown Square, approximately 6 miles from the hospital. Lankenau Hospital, established in 1860, is a community-based 331-bed acute care teaching hospital located in Wynnewood, Pennsylvania. Lankenau offers a wide array of cutting-edge clinical specialties and is know nationally for its acclaimed Heart Center. Other programs include a state-of-the-art cancer center, a Level III Neonatal Intensive Care Unit, a kidney transplant program, and an accredited sleep disorders center. Lankenau provides outpatient ambulatory care services in Broomall, Pennsylvania, approximately 7 miles from the hospital. Paoli Hospital is a 226-bed community-based, acute care hospital, established in 1913 and located in Paoli, Pennsylvania. In addition to a full range of community hospital programs, Paoli Hospital became a designated Level II Trauma Center on October 1, 2010, the only trauma center in Chester County. Paoli Hospital also provides a comprehensive range of cardiac services, outpatient psychiatric and addictive diseases services, interventional radiology, a full range of imaging services, a breast and comprehensive cancer center in partnership with Fox Chase Cancer Center, maternity services, and a large orthopedic program. To accommodate community needs, a new four-story, 281,000 square foot Patient Care Pavilion with 124 private rooms, 14 state-of-the-art operating rooms and an emergency department three times the size of the former emergency department, opened in June 2009. In addition, three Paoli ambulatory sites extend services into the western (Exton, approximately 8 miles from the hospital) and the northern (Collegeville, 12 miles from the hospital, and Audubon, 11 miles from the hospital) markets. Bryn Mawr Rehabilitation Hospital (\"Bryn Mawr Rehab\") is a 148-bed specialty hospital for physical medicine and rehabilitation located in Malvern, Pennsylvania. Opened in 1970, Bryn Mawr Rehab provides treatment to more than 2,700 individuals annually who have experienced physical and cognitive illnesses or injuries. Bryn Mawr Rehab offers inpatient, outpatient and transitional programs for individuals with brain injury, stroke, spinal cord injury, multiple sclerosis, orthopedic injuries and other neurological and musculoskeletal conditions. IV. Riddle Memorial Hospital Riddle Memorial Hospital, established in 1963, is a community-based, 230-bed acute care hospital located in Media, Pennsylvania. Effective February 28, 2007, Riddle Memorial Hospital formally affiliated with Main Line Health. Although it operates in coordination with Bryn Mawr, Lankenau and Paoli hospitals, the acute care hospitals that are operating divisions on Main Line Hospitals, Inc., Riddle Memorial Hospital is a separate Pennsylvania nonprofit corporation. Like Main Line Hospitals, Inc., the sole corporate member of Riddle Memorial Hospital (\"Riddle Hospital\") is Main Line Health. In addition to a full range of community hospital programs, Riddle Hospital provides services in interventional radiology, a cardiac catheterization program, occupational medicine, a comprehensive cancer center in association with Thomas Jefferson University Hospital's radiation therapy program, a full range of imaging services, a breast health center, maternity services, and a highly recognized orthopedic center of excellence. Riddle Memorial Hospital is the sole corporate member of Mirmont Treatment Center, which operates a 102-bed alcohol and substance abuse rehabilitation center in Lima, Pennsylvania. V. Lankenau Institute for Medical Research Lankenau Institute for Medical Research (\"LIMR\") is a Pennsylvania nonprofit corporation located on the campus of Lankenau Medical Center. The sole member of LIMR is Main Line Hospitals, Inc. LIMR operates a medical research center concentrating on bench to bedside research on diseases of aging, particular cancer and heart disease. LIMR-based research scientists and physicians currently participate in more than 300 clinical research projects. VI. Other MLH Provider Entities 1. Main Line Affiliates is a Pennsylvania nonprofit corporation which provides rehabilitation services, including therapist staff and management services, to support all Main Line Health hospitals and provide home care rehabilitation services to support Bryn Mawr Rehab. Services are provided to patients in hospitals, skilled nursing facilities, assisted living facilities and home settings. 2. The Home Care Network (\"THCN\"), a Pennsylvania nonprofit corporation, provides home health care, hospice and palliative care and private duty nursing services to the community and participates in Medicare and Medicaid programs. 3. The Hospice and Home Care Foundation is a Pennsylvania nonprofit corporation which engages in fundraising activities in support of THCN's home care and hospice programs of Main Line Health and its nonprofit subsidiaries. 4. Main Line Health Laboratories (\"MLHL\") is a Pennsylvania nonprofit, nonstock corporation. MLHL is the entity through which Main Line Health's clinical laboratories are consolidated and operated. VII. Related Entities Main Line Health, directly or through a subsidiary, owns interests in other entities that complement its health care mission. These include: 1. Main Line Health Real Estate, L.P., a joint venture with Lankenau Hospital Foundation, which holds certain real estate assets for investment purposes. 2. Main Line Health Imaging, L.P., a joint venture which owns and operate imaging centers located at, adjacent to or near Main Line Health facilities. 3. Riddle Surgical Center, L.P., a joint venture which owns and operates an ambulatory surgery center on Riddle's campus. 4. Riddle Hospital's service area is anticipated to grow by 4.1 percent while that of Bryn Mawr Hospital is expected to see no change. Lankenau's service area is anticipated to decrease in population by approximately 1.8 percent. Socio-Demographics: The four county area tends to be upwardly mobile although there are significant variations and pockets of poverty. Generally, the area enjoys higher household incomes, more education and higher median income. In total, Bryn Mawr Rehabilitation Hospital received approximately 36 percent of net revenue from government programs in FY2010. The loss incurred in providing healthcare services to patients of government programs approximated $2,305,013 as detailed on Schedule H, Part I, line 7b(e) and Part III, line 2. For all of Main Line Health's service areas, however, the largest current age segment is 18 to 44 years of age followed by the 45 to 64-year group. However, for all of its service areas, the fastest growing age segment is 65 years and above. For Paoli and Riddle, the aging of their populations will be the most pronounced because of the current age group concentrations. Those two groups (ages 18 to 44 and 45 to 64) will be aging in greater numbers over the next ten years. Lankenau's service area is more representative of southeastern Pennsylvania which exhibits greater diversity. Conversely, the African American population of Lankenau is 19.4 percent while that of Bryn Mawr is 4.4 percent, Paoli, 3.6 percent and Riddle 5.3 percent. For southeastern Pennsylvania, the African American population is 20.6 percent. The Hispanic population is greatest for Riddle with 3.1 percent. The other hospitals range from 2.5 percent at Bryn Mawr, 2.6 percent at Paoli and 2.7 percent at Lankenau. However, it is important to recognize that the income levels are not evenly distributed in the communities we serve. In fact, the presence of some extreme high-income neighborhoods tends to obscure the presence of low to moderate income neighborhoods whose residents may need services. Community Characteristics - The high percentage of college educated adults is an asset for MLH communities. A high level of homeownership also indicates a high degree of stability in the area. Geography - The MLH service area touches upon a wide range of communities. Background: In September 2009, Main Line Health (MLH) completed a formal Community Health Needs Assessment. The results of the assessment, which included input from community-based healthcare providers as well as elected officials, indicated that there were no unknown issues among the various stakeholders. The assessment also pointed out several \"known\" issues that are part of a national trend, including obesity and non-compliance for certain age/cultural groups for both mammograms and colonoscopies. The need for a thorough community health assessment was recognized by the senior leadership team of Main Line Health and included as a tactic in its strategic plan which was shared with all employees in 2008. Preliminary planning for the assessment began later that year with all research completed by early 2010. The goals were to: 1- Ensure that gaps in the health care continuum were met, duplication is avoided through collaboration, and efficiencies and effectiveness in care delivery are improved. 2-Demonstrate a proactive approach to coordinating community and charity care 3-Exemplify public health leadership within the region. Knowing that baseline information, trends, needs and analysis needed to be met, Main Line Health senior leadership developed an overarching strategy: Align MLH and its physicians with the healthcare needs of the populations it serves. Demonstrating these needs to the physician community was a key undertaking because of the approximate 1,900 physicians who practice at Main Line Hospitals, only 250 are employed. The community-based physicians not only were a resource for information but would be a partner and conduit for ultimate solutions and initiatives. With this overarching strategy in mind, several tactics became apparent: 1-An assessment would be conducted that would utilize a process to engage community leaders/elected officials, other nonprofit organizations, physicians and employees. 2-MLH would determine which relevant services would be implemented as determined by the community needs assessment within economic feasibility, as well as in selected geographic areas by ultimately partnering with physician groups, insurance companies and/or other community providers. 3-The results would be leveraged to secure appropriate (and additional) sources of financial and community support 4-MLH would measure and report actionable results to the community. The assessment process spanned several months. Data sources included: Thomson-Reuters, including Census data, Philadelphia Health Management Corporation's (PHMC) southeastern Pennsylvania Household Health Survey, MLH proprietary surveys of healthcare providers and community stakeholders, including elected officials, United Way data, information from the Hospital and Health Ssystem Association of Pennsylvania, as well as other industry-based sources. The geography generally covers the basic service area, both primary and secondary as determined by percent of discharges, of Main Line Health hospitals, the primary and secondary zip codes of each MLH acute-care hospital, and benchmark data where available from southeastern Pennsylvania (SEPA) or Pennsylvania (PA). Results: Ten primary issues were identified through the assessment for potential action, although many other issues were studied. These were highlighted because the MLH area demonstrated risk at or above the level of regional or state benchmarks, the issue reflected a national trend, accurate data were available and validated the needs, and stakeholder concerns. Those ten community health issues in the Main Line Health area identified were: 1-Meeting the needs of a growing senior population 2-Changing demographics-emergency and maternity care impact 3-Lifestyle issues-smoking, obesity, sleep and stress 4-Chronic conditions - helping manage the burden of disease 5-Heart disease and stroke 6-Early detection of cancer 7-Concerns about the health of children and youth 8-Mental and behavioral health - expanding access for those in need 9-The cost of health care 10-Tapping into - and coordinating with - community resources. It should be noted that during the timeframe of this needs assessment, the United States, Pennsylvania, and specifically, the communities in Montgomery, Delaware and Chester Counties where Main Line Health facilities are located, were in the midst of a deep recession. While the economic indicators had a significant impact on the results of the assessment, each of the 10 key issues has its own set of economic ramifications. In addition, toward the end of the assessment, healthcare reform began to become a topic of discussion. While those discussions did not impact the surveys directly, in conjunction with the economic situation uncertainty about health care in general was an underlying theme. Main Line Health intends to utilize this 2009 community health needs assessment as a baseline and begin a regular series of such studies every other year. The next assessment is planned for late 2011 with results to be published in 2012.","other_information":"When Main Line Health (MLH) was formed in 1985, the long term objectives included the desire of the community-based boards to more fully serve the individual communities by providing greater efficiencies and by increasing the broader outreach of the hospitals. Membership in the governing board is monitored closely to ensure representation in socio-demographics and across subject matter expert areas that would be expected and needed to provide the guidance and oversight of an integrated healthcare delivery system. Members include executives with expertise in health care quality, finance, human resources, law, technology, marketing and manufacturing as well as healthcare. The Board is focused on its responsibility to provide excellent care to the community while generating funds for reinvestment in technology, equipment and facilities. Foremost, they are adamant that quality and patient safety not be compromised, particularly as healthcare providers are challenged by: - financial ramifications of the economy, - the impact of demographics, - the uncertainty relating to future levels of reimbursement, and - the growing expectations of an aging population that the best healthcare will be \"there\" for them. Each of the programs has a set of criteria which must be met for the patient to qualify for healthcare financial assistance. Financial Counselors MLH provides financial counselors at each hospital to help patients find ways to meet their financial obligations for the healthcare services provided to them. Specifically, financial counselors help patients apply for government assistance programs as well as MLH Charity Care and Financial Assistance. Any patient may request to speak to a financial counselor when being scheduled for treatment at MLH. Uninsured patients who are admitted to the hospital will automatically receive help from a financial counselor. There are several options available to patients without insurance. They are: Option 1: Government Sponsored Medical Assistance There are a number of available programs from the Federal, State and Local levels such as Medical Assistance which can help pay the medical bills of people who have low income and cannot afford medical care. Often, patients are unaware of these programs or are unable to access them due to the cumbersome enrollment process required to receive these benefits. Financial Counselors can help patients apply for Medical Assistance to determine eligibility. Our financial counselors will assist in the application process and follow through with the governmental agency to assist the patient. Option 2: Charity Care MLH offers Charity Care/Financial Assistance to uninsured patients that need care. For patients with income levels at 200% or less of the Federal Poverty Guidelines, 100% Charity Care (free care) is provided. For patients with income between 200% and 500% of the Federal Poverty Guidelines significant charity discounts (75% - 65%) are provided. MLH Financial Counselors assist patients with the Charity Care/Financial Assistance application. Option 3: Uninsured Patient Discount For Uninsured Patients whose income is in excess of 500% of the Federal Poverty Guidelines, a substantial discount based on the following guidelines is provided: - For all Inpatient Non-Elective services a discount of 60% off total charges will be applied. - For all Inpatient Elective services a discount of 40% off total charges will be applied. - For all Emergency Department services, those treated and released, a discount of 40% off total charges will be applied with a maximum charge of $1,000 for the Emergency Department visit. - For all other Outpatient services a discount of 40% off total charges. MLH applies the Uninsured Patient Discount on a consistent and non-discriminatory basis. The MLH Charity Care & Financial Assistance Policy is made available to the public on the MLH website. Extended Payment Arrangements to Patients If patients are unable to pay the patient responsibility portion of their bill at the time of service or at the time of discharge, payment options for extended, interest free payment periods are available. Charges for services rendered to patients that meet MLH's guidelines for charity care are not reflected in the accompanying financial statements. Discounts offered for financial assistance are reflected as reductions of net patient revenue. MLH maintains records to identify and monitor the level of charity care provided. Management estimates that the cost associated with these services for charity care provided by Bryn Mawr Rehabilitation Hospital approximated $13,009 for the fiscal year ended June 30, 2010. Services are designed to build strength and endurance, restore functional mobility, improve performance of daily activities and manage pain. More than 2,000 patients receive inpatient services each year at Bryn Mawr Rehabilitation Hospital for joint replacements, fractures, and amputations. They range in age from 18 to 97 years old. The average stay is eight days and ranges from 5 to 14 days. More than 90% of our inpatients return home after discharge. As a comprehensive rehabilitation facility, our services also include outpatient care for the treatment of strains, sprains, bursitis and sports injuries. We have developed a full continuum of lifetime care for people who have experienced a stroke. This system includes acute inpatient services, outpatient services, Day Treatment, community re-entry services, and Family Support Groups. When stroke survivors arrive at Bryn Mawr Rehabilitation Hospital, they usually require total or maximal assistance with daily activities. Our medical care, therapies, and special services help each person build on strengths and compensation for any limitations. Main Line Hospitals, Inc. is the surviving entity of the 1996 merger of Lankenau Hospital and Paoli Memorial Hospital into Bryn Mawr Hospital. The Bryn Mawr Hospital corporation's name was changed following the merger to Main Line Hospitals, Inc. Effective July 1, 2010, Bryn Mawr Rehabilitation Hospital, formerly a separate Pennsylvania nonprofit corporation was merged into Main Line Hospitals, Inc. At present, Main Line Hospitals, Inc. operates Lankenau Hospital, Paoli Hospital and Bryn Mawr Hospital as separately licensed acute care hospitals, and Bryn Mawr Rehabilitation Hospital as a separately licensed acute rehabilitation hospital. Bryn Mawr Hospital, Lankenau Hospital, Paoli Hospital and Bryn Mawr Rehabilitation Hospital are governed by a Main Line Health board of governors consisting of 27 members of the community, including physician representation. The Board has overall governance and accountability patient safety and quality, and community outreach. Riddle Memorial Hospital has a separate 16member board of directors comprised primarily of community members, MLH management, and physicians. Each MLH hospital has an associated nonprofit Foundation engaged in fundraising and development. The Boards of each foundation consist of community representatives who provide oversight and guidance for the Foundations philanthropic activities. Bryn Mawr Hospital, established in 1893, is a community-based 311-bed acute care teaching hospital located in Bryn Mawr, Pennsylvania. Bryn Mawr Hospital offers a full complement of medical and surgical services, as well as specialized programs such as primary care services, an arthritis and orthopedic center, a comprehensive cancer car center, a cardiovascular center, and women's and children's health services. Bryn Mawr Hospital has the only in-patient psychiatric facility in Montgomery County as well as the only emergency department specific for children. The Bryn Mawr Hospital Health Center provides outpatient ambulatory care services in Newtown Square, approximately 6 miles from the hospital. Lankenau Hospital, established in 1860, is a community-based 331-bed acute care teaching hospital located in Wynnewood, Pennsylvania. Lankenau offers a wide array of cutting-edge clinical specialties and is know nationally for its acclaimed Heart Center. Other programs include a state-of-the-art cancer center, a Level III Neonatal Intensive Care Unit, a kidney transplant program, and an accredited sleep disorders center. Lankenau provides outpatient ambulatory care services in Broomall, Pennsylvania, approximately 7 miles from the hospital. Paoli Hospital is a 226-bed community-based, acute care hospital, established in 1913 and located in Paoli, Pennsylvania. In addition to a full range of community hospital programs, Paoli Hospital became a designated Level II Trauma Center on October 1, 2010, the only trauma center in Chester County. Paoli Hospital also provides a comprehensive range of cardiac services, outpatient psychiatric and addictive diseases services, interventional radiology, a full range of imaging services, a breast and comprehensive cancer center in partnership with Fox Chase Cancer Center, maternity services, and a large orthopedic program. To accommodate community needs, a new four-story, 281,000 square foot Patient Care Pavilion with 124 private rooms, 14 state-of-the-art operating rooms and an emergency department three times the size of the former emergency department, opened in June 2009. Bryn Mawr Rehabilitation Hospital (\"Bryn Mawr Rehab\") is a 148-bed specialty hospital for physical medicine and rehabilitation located in Malvern, Pennsylvania. Opened in 1970, Bryn Mawr Rehab provides treatment to more than 2,700 individuals annually who have experienced physical and cognitive illnesses or injuries. Bryn Mawr Rehab offers inpatient, outpatient and transitional programs for individuals with brain injury, stroke, spinal cord injury, multiple sclerosis, orthopedic injuries and other neurological and musculoskeletal conditions. IV. Riddle Memorial Hospital Riddle Memorial Hospital, established in 1963, is a community-based, 230-bed acute care hospital located in Media, Pennsylvania. Effective February 28, 2007, Riddle Memorial Hospital formally affiliated with Main Line Health. Although it operates in coordination with Bryn Mawr, Lankenau and Paoli hospitals, the acute care hospitals that are operating divisions on Main Line Hospitals, Inc., Riddle Memorial Hospital is a separate Pennsylvania nonprofit corporation. Like Main Line Hospitals, Inc., the sole corporate member of Riddle Memorial Hospital (\"Riddle Hospital\") is Main Line Health. In addition to a full range of community hospital programs, Riddle Hospital provides services in interventional radiology, a cardiac catheterization program, occupational medicine, a comprehensive cancer center in association with Thomas Jefferson University Hospital's radiation therapy program, a full range of imaging services, a breast health center, maternity services, and a highly recognized orthopedic center of excellence. Riddle Memorial Hospital is the sole corporate member of Mirmont Treatment Center, which operates a 102-bed alcohol and substance abuse rehabilitation center in Lima, Pennsylvania. V. Lankenau Institute for Medical Research Lankenau Institute for Medical Research (\"LIMR\") is a Pennsylvania nonprofit corporation located on the campus of Lankenau Medical Center. The sole member of LIMR is Main Line Hospitals, Inc. LIMR operates a medical research center concentrating on bench to bedside research on diseases of aging, particular cancer and heart disease. LIMR-based research scientists and physicians currently participate in more than 300 clinical research projects. VI. Other MLH Provider Entities 1. Main Line Affiliates is a Pennsylvania nonprofit corporation which provides rehabilitation services, including therapist staff and management services, to support all Main Line Health hospitals and provide home care rehabilitation services to support Bryn Mawr Rehab. Services are provided to patients in hospitals, skilled nursing facilities, assisted living facilities and home settings. 2. The Home Care Network (\"THCN\"), a Pennsylvania nonprofit corporation, provides home health care, hospice and palliative care and private duty nursing services to the community and participates in Medicare and Medicaid programs. 3. The Hospice and Home Care Foundation is a Pennsylvania nonprofit corporation which engages in fundraising activities in support of THCN's home care and hospice programs of Main Line Health and its nonprofit subsidiaries. 4. Main Line Health Laboratories (\"MLHL\") is a Pennsylvania nonprofit, nonstock corporation. MLHL is the entity through which Main Line Health's clinical laboratories are consolidated and operated. VII. Related Entities Main Line Health, directly or through a subsidiary, owns interests in other entities that complement its health care mission. These include: 1. Main Line Health Real Estate, L.P., a joint venture with Lankenau Hospital Foundation, which holds certain real estate assets for investment purposes. 2. Main Line Health Imaging, L.P., a joint venture which owns and operate imaging centers located at, adjacent to or near Main Line Health facilities. 3. Riddle Surgical Center, L.P., a joint venture which owns and operates an ambulatory surgery center on Riddle's campus. 4. Lankenau's service area is anticipated to decrease in population by approximately 1.8 percent. Socio-Demographics: The four county area tends to be upwardly mobile although there are significant variations and pockets of poverty. Generally, the area enjoys higher household incomes, more education and higher median income. In total, Bryn Mawr Rehabilitation Hospital received approximately 36 percent of net revenue from government programs in FY2010. The loss incurred in providing healthcare services to patients of government programs approximated $2,305,013 as detailed on Schedule H, Part I, line 7b(e) and Part III, line 2. For all of Main Line Health's service areas, however, the largest current age segment is 18 to 44 years of age followed by the 45 to 64-year group. However, for all of its service areas, the fastest growing age segment is 65 years and above. For Paoli and Riddle, the aging of their populations will be the most pronounced because of the current age group concentrations. Those two groups (ages 18 to 44 and 45 to 64) will be aging in greater numbers over the next ten years. Community Characteristics - The high percentage of college educated adults is an asset for MLH communities. A high level of homeownership also indicates a high degree of stability in the area. Geography - The MLH service area touches upon a wide range of communities. Background: In September 2009, Main Line Health (MLH) completed a formal Community Health Needs Assessment. The results of the assessment, which included input from community-based healthcare providers as well as elected officials, indicated that there were no unknown issues among the various stakeholders. The assessment also pointed out several \"known\" issues that are part of a national trend, including obesity and non-compliance for certain age/cultural groups for both mammograms and colonoscopies. The need for a thorough community health assessment was recognized by the senior leadership team of Main Line Health and included as a tactic in its strategic plan which was shared with all employees in 2008. Preliminary planning for the assessment began later that year with all research completed by early 2010. The goals were to: 1- Ensure that gaps in the health care continuum were met, duplication is avoided through collaboration, and efficiencies and effectiveness in care delivery are improved. 2-Demonstrate a proactive approach to coordinating community and charity care 3-Exemplify public health leadership within the region. Knowing that baseline information, trends, needs and analysis needed to be met, Main Line Health senior leadership developed an overarching strategy: Align MLH and its physicians with the healthcare needs of the populations it serves. Demonstrating these needs to the physician community was a key undertaking because of the approximate 1,900 physicians who practice at Main Line Hospitals, only 250 are employed. The community-based physicians not only were a resource for information but would be a partner and conduit for ultimate solutions and initiatives. With this overarching strategy in mind, several tactics became apparent: 1-An assessment would be conducted that would utilize a process to engage community leaders/elected officials, other nonprofit organizations, physicians and employees. 2-MLH would determine which relevant services would be implemented as determined by the community needs assessment within economic feasibility, as well as in selected geographic areas by ultimately partnering with physician groups, insurance companies and/or other community providers. 3-The results would be leveraged to secure appropriate (and additional) sources of financial and community support 4-MLH would measure and report actionable results to the community. The assessment process spanned several months. Data sources included: Thomson-Reuters, including Census data, Philadelphia Health Management Corporation's (PHMC) southeastern Pennsylvania Household Health Survey, MLH proprietary surveys of healthcare providers and community stakeholders, including elected officials, United Way data, information from the Hospital and Health Ssystem Association of Pennsylvania, as well as other industry-based sources. The geography generally covers the basic service area, both primary and secondary as determined by percent of discharges, of Main Line Health hospitals, the primary and secondary zip codes of each MLH acute-care hospital, and benchmark data where available from southeastern Pennsylvania (SEPA) or Pennsylvania (PA). Results: Ten primary issues were identified through the assessment for potential action, although many other issues were studied. These were highlighted because the MLH area demonstrated risk at or above the level of regional or state benchmarks, the issue reflected a national trend, accurate data were available and validated the needs, and stakeholder concerns. Those ten community health issues in the Main Line Health area identified were: 1-Meeting the needs of a growing senior population 2-Changing demographics-emergency and maternity care impact 3-Lifestyle issues-smoking, obesity, sleep and stress 4-Chronic conditions - helping manage the burden of disease 5-Heart disease and stroke 6-Early detection of cancer 7-Concerns about the health of children and youth 8-Mental and behavioral health - expanding access for those in need 9-The cost of health care 10-Tapping into - and coordinating with - community resources. It should be noted that during the timeframe of this needs assessment, the United States, Pennsylvania, and specifically, the communities in Montgomery, Delaware and Chester Counties where Main Line Health facilities are located, were in the midst of a deep recession. While the economic indicators had a significant impact on the results of the assessment, each of the 10 key issues has its own set of economic ramifications. In addition, toward the end of the assessment, healthcare reform began to become a topic of discussion. While those discussions did not impact the surveys directly, in conjunction with the economic situation uncertainty about health care in general was an underlying theme. Main Line Health intends to utilize this 2009 community health needs assessment as a baseline and begin a regular series of such studies every other year. The Board is focused on its responsibility to provide excellent care to the community while generating funds for reinvestment in technology, equipment and facilities. Foremost, they are adamant that quality and patient safety not be compromised, particularly as healthcare providers are challenged by: - financial ramifications of the economy, - the impact of demographics, - the uncertainty relating to future levels of reimbursement, and - the growing expectations of an aging population that the best healthcare will be \"there\" for them. They are: Option 1: Government Sponsored Medical Assistance There are a number of available programs from the Federal, State and Local levels such as Medical Assistance which can help pay the medical bills of people who have low income and cannot afford medical care. Often, patients are unaware of these programs or are unable to access them due to the cumbersome enrollment process required to receive these benefits. Financial Counselors can help patients apply for Medical Assistance to determine eligibility. Our financial counselors will assist in the application process and follow through with the governmental agency to assist the patient. Option 2: Charity Care MLH offers Charity Care/Financial Assistance to uninsured patients that need care. For patients with income levels at 200% or less of the Federal Poverty Guidelines, 100% Charity Care (free care) is provided. For patients with income between 200% and 500% of the Federal Poverty Guidelines significant charity discounts (75% - 65%) are provided. MLH Financial Counselors assist patients with the Charity Care/Financial Assistance application. Option 3: Uninsured Patient Discount For Uninsured Patients whose income is in excess of 500% of the Federal Poverty Guidelines, a substantial discount based on the following guidelines is provided: - For all Inpatient Non-Elective services a discount of 60% off total charges will be applied. - For all Inpatient Elective services a discount of 40% off total charges will be applied. - For all Emergency Department services, those treated and released, a discount of 40% off total charges will be applied with a maximum charge of $1,000 for the Emergency Department visit. - For all other Outpatient services a discount of 40% off total charges.
