Form 990, Part VI, Line 11 Process for Review of Form 990 The Plan prepares Form 990 with the assistance of its independent accounting firm KPMG, who signs the return as paid preparer. The return is then reviewed by the finance director and submitted to the Board before it is filed with the IRS. Form 990, Part VI, Line 12 Conflict of Interest Policy Monitoring Board members and officers are required annually to disclose conflict of interest in a form prescribed by the Board. The conflict statements are reviewed by the Board. In the event of any conflict, the Board member will recuse him or herself from any deliberations regarding the transaction. Form 990, Part VI, Lines 15a & 15b Process for Determining Compensation Salary was determined from comparable data with similar companies in our healthcare field, and a complete analysis of job duties and job description. The compensation is provided by VCU Health System Authority. Carolina Crescent does not compensate its officers or directors other than the Executive Director.
