Each apartment is fully furnished, and the team assists the individual with move-in and orientation to the apartment, building, and community.act services: after receiving housing, individuals are assigned to one of eight assertive community treatment (act) teams, multi-disciplinary teams offering psychiatric, medical, social, and other intensive support services. Our act teams meet with their assigned participants individually at least six times per month, using the individual's comprehensive service plan to identify the client's own goals for recovery and reintegration into the community (e.g., employment, smoking cessation, medications). The team's role is to support the individual in meeting his or her goals.supported housingpathways also has a supported housing program for participants who have \"graduated\" from act and do not require intensive treatment services. Selected individuals engage in a six-month training program to develop the skills necessary to live with an even greater level of independence.
