About 15% were uninsured or underinsured and the majority reside in the underserved urban communities of birmingham and fairfield, alabama. For our patient population we provided free dental care to 95 patients. Services were provided by licensed volunteer dentists, hygenists & dental assistants (a total of 10 volunteers). Also during this fiscal year 92 of our patients received counseling from our licensed counselor, who provided 148 counseling sessions. We also provide free diet counseling and exercise classes every saturday at our clinic. Almost 2,900 of the patient encounters were for primary health care provided by our two board- certified medical doctors. We continued to employ a part-time licensed counselor to provide free counseling services (over 200 patient encounters) for our primary care patients. We also continued to provide dental services at a nominal fee using volunteer dentists and hygienists until april, 2015. At that point, we were able to hire a part-time (two days a week) dentist and dental auxiliary to provide dental services at prices that are affordable for patients in the community we serve. We continued to offer free counseling services and group activities led by our licensed counselor two days week. However, counseling patient encounters of about 290 were down 34% versus fy 2017-18 due primarily to a three-month suspension of services resulting from our counselor's medical leave. Finally, we continued to offer affordable eye care services about two afternoons a month using a third- party provider whose mission is complementary with our mission. We had about 290 vision patient encounters (up 22% vs. Fy 2017-18). Across the services we offer, about 87% of the patients were poor,elderly, disabled, and/or uninsured. Other significant activities in the community included distribution of about 125 thanksgiving baskets, our sixth annual christmas shop, and free health screenings for students (pre-k and high school) from local schools. Finally, one of our medical doctors led a medical mission team that saw over 1,000 patients in gonaives, haiti. the fiscal year ended june 30, 2021, we continued to provide primary care, counseling, and eye care services based out of our clinic in fairfield, alabama. The patient population continued to consist largely of the elderly, disabled, and/or uninsured. Primary health care, which is provided by our two board-certified medical doctors,accounted for about 3,350 patient encounters. That is up about 15% versus the previous year largely because of allmost 600 covid vaccines that were administered. The patient encounteres were largely in person visits. Our licensed counselor continued to work remotely due to covid-19. She provided individual counseling sessions about one and one-half days per week and a weekly group bible study. We continued to offer affordable eye care services about two afternoons a month using a third-party provider whose mission is complementary with our mission. Finally, medical mission trips to haiti and cote d'ivoire remained suspended due to covid-19 and/or social unrest in those countries. However, we have remained in touch with our ministry partners in those locaitons and have provided some financial assistance. Other significant activities in the community included distribution of about 100 thanksgiving baskets, our eighth annual christmas shop, and free pre-k student health screenings. After thoughtful and prayerful consideration by our board, we received a paycheck protection program loan of about 80,000 from the U.S.. Government during fy 2019-20. january 26, 2023, we transferred substantially all of our non-financial assets to a larger medical practice (a 501(c)(3) organization-see below) with a complementary mission. Following the transfer of assets, we worked with our legal, accounting and tax advisors/consultants to dissolve the corporate entity. Funds remaining at dissolution were transferred to a 501(c)(3) organization - national christian foundation alabama -- via a board approved grant. Through january 20, 2023 (six days before the asset transfer), we continued to provide primary care, counseling and eye care services based out of our clinic in fairfield, alabama. The patient population consisted largely of the elderly, disabled, and/or uninsured. Other significant activities in the fairfield community included distribution of about 125 thanksgiving baskets, our tenth annual christmas shop, and free pre-k student health screenings. After january 20, 2023, we no longer accepted contributions from donors, no longer had employees, and no longer provided health care services. The clinic we formerly owned and operated reopened on february 6, 2023 as cahaba medical care - hope health, which is part of a federally qualified health care (fqhc) network and a 501(c)(3)organization.
